Check-In & Day-of Requirements
Check-in is online on GotSport only and must be completed by May 18, 2026 at 5pm.
Please note: all team documents must be from the same governing body, it is not permissible to mix USYS and USClub documents.
Check-in is on GotSport through your KISS registration.
Please see below for tournament roster help, all forms, and uploading instructions as needed.
Roster & Forms needed on GotSport:
- Create a tournament roster.
- Upload KISS Liability Form.
- If applicable, upload Guest Player Forms.
- If applicable, upload Permission to Travel Form*.
*USClub teams do not need a permission to travel form.
Required to have with team at all times during the tournament:
- Medical Release Form for each player. As appropriate for team’s governing body (USYS or USClub).
- CDC Parent Player Concussion Form for each player.
- Player and Team Officials Passcards for all participating players.
- Risk Management cards for all Team Officials.
- SafeSport Certifications for all Team Officials.
